How CHW Support & Referral Assistance Works
1. Submit a Referral or Request Support
A provider, organization, CHW, student, individual, or family member completes a referral or request form with basic information about the need.
2. Initial Review
IMI Management reviews the request to better understand the situation, referral need, or partnership opportunity.
3. Resource or Service Connection
When appropriate, we help identify possible resources, workshops, support services, or referral pathways that may fit the need.
4. Follow-Up When Appropriate
If follow-up is part of the service request or partnership arrangement, IMI Management may provide support to help clarify next steps, encourage connection, or identify additional needs.
5. Partnership or Workshop Planning
For providers, schools, nonprofits, and organizations, IMI Management can also discuss workshops, referral processes, CHW support services, or community partnership opportunities.
